Featuring an 84,432-square-foot hospital expansion and a 30,000-square-foot medical office building, the campus stands as the largest Federally Qualified Health Center site in Ohio.

Hplex Solutions proudly served as Owner’s Representative for the development of Five Rivers Health Centers – Xenia Campus, a 14,575-square-foot medical office building designed to expand access to essential healthcare services in Xenia, Ohio. The project involved renovating an existing retail building into a healthcare facility tailored to support a wide range of outpatient services. As Five Rivers Health Centers outgrew two existing spaces, this project gave them the opportunity to combine and expand services under one roof, creating a better and more seamless patient experience.

Hplex Solutions proudly served as the Owner’s Representative for the development of the Fond du Lac Community Hospital. Designed under an innovative community hospital model, this new facility provides critical emergency and inpatient healthcare services within a smaller, highly efficient footprint. This project reflects a shared commitment by Froedtert ThedaCare Health to expand convenient access to high-quality, patient-centered care for the growing community of Fond du Lac, Wisconsin.

Hplex Solutions was deeply honored to serve as the Owner’s Representative for the development of Dani’s Place, a pioneering mental health rehabilitation center. Located in Toledo, it provides a critical bridge in the care continuum, offering up to 90 days of housing and support for individuals transitioning from inpatient psychiatric care. This facility stands as a testament to what is possible when a community unites to address a critical need.

Hplex Solutions served as the developer and continues as property manager for Walnut Creek Wellness Center, a modern medical office building completed in 2018. The 30,056-square-foot, two-story facility brings together six healthcare tenants, offering primary care, OBGYN, orthopedics, plastic surgery, aesthetics, and lab services, within a cohesive and contemporary environment.

Hplex Solutions proudly served as the Owner’s Representative for the development of Mount Carmel Dublin, a state-of-the-art hospital and medical campus that opened in April 2025. The project features a 246,000-square-foot, 60-bed hospital and 14-bed emergency department, providing a full range of inpatient, outpatient and specialty services including primary care, orthopedics, diagnostic services, and surgical specialties.
